Conducting a root cause analysis is designed to find the cause of pressure injury, but there is more to know before a plan of correction can be developed. Questions in the analysis include when did the wound start and could anything have been done differently at that time. Also, what position was the patient in when the patient was injured and was repositioning possible? How does the forensics on the wound match the documentation of care provided? This session will help identify the complete story to strengthen plans for education and enhance documentation.
